NEW YORK: Broome, Tioga, Chenango, Tompkins, Chemung, Schuyler, and Steuben counties.
PENNSYLVANIA: Susquehanna and Bradford counties.

STEPS:

- Save the file to your computer by pressing the PRINT icon **Then save as PDF file**.
- Send Options:
- Upload the PDF file that is filled out with your information to the form below.
- Print out the form: Mail or Fax it to the address on the cover sheet.
